That's on the Classics. On the -500 and -600, the condition levers are generally left in the AUTO position for take-off and landing.

A possibility is that they may have been too high...

A possibility is that they may have been too high to change runway and decided to put the engines into 100% over-ride (fine pitch) using the condition levers to create drag and maybe move them back...

Really horrific. The crash axe is normally behind the FO seat low down near the gear pins (on 72-600). With oxy masks on and full of water, I can't imagine how difficult it would be to retrieve....

The most important thing to say about this is that the flight crew were sloppy right from the start. They were not ever told to climb to FL020. It was FL200. Add two zeros ....that's 20,000 feet on...
