Well flying the 737 my take on your question is your commanded engine RPM tends to increase as you go higher till it reaches a max value.. generally reaches about 99-100%(104% being the max).

Im a trainee on the 738.. pre-sim stage .. so kindly excuse me if this question has a really obvious answer, but what is the use of the Vref+15 white bug? Since we maintain only Vref+5 generally...
