Haven't heard the show yet, but expect they're tlking about "Significant Event Analysis."

It's something positive that we picked up from the aviation industry. In our surgery, we partake in the process weekly, and it's infinately better than what we used to do - (standing at the bar saying "phew, had a close one today.")

Of course, building stuff into the system to stop us mere humans making mistakes in the first place, also helps.

Thanks for the hint Genghis, recorded it yesterday as I recently happen to be in research on safety culture (in medicine, not in aviation unfortunately, but you can't win them all :E).

FWIW, concerning the role of design on error occurrence (in an aviation/industry context), the studies I am currently aware of are a journal article from 2007 (Kinnersley & Roelen, The contribution of design to accidents 10.1016/j.ssci.2006.08.010 : Safety Science | ScienceDirect.com) and several research papers by Asaf Degani which are available online, quite interesting reading.
