A memo has been sent to all Batik/Lion pilots advising of Unreliable airspeed issue just in the last hour.

The wrong application of a recall item in a perfectly good aircraft hopefully not.

Statement by Lion Air on crew

The aircraft is commanded by Captain. Bhavye Suneja and co-pilot Harvino with six cabin crew Shintia Melina, Citra Noivita Anggelia, Alviani Hidayatul Solikha, Damayanti Simarmata, Mery Yulianda, and Deny Maula. The captain has 6,000 flight hours and the co-pilot has more than 5,000 flight hours.

If just on pitot was faulty, airspeed & stick shaker warnings will operate until it is determined which one needs to be disabled. This should leave two to work with. How quickly the faulty one can be determined is an interesting question.
I hope the pilot in control was flying the plane rather than trying to diagnose the issue.

Looking at the flight on FlightAware, the speed and altitude are quite unstable from a minute or two into the flight.

Less than two minutes into the flight speed increases and altitude decreases, then reverses again to climb, the rates are all over the place - looks like they were trying to hold 5,000 feet. About ten minutes into the flight, according to FlightAware ADS-B, they start down at -1,114 fpm, then -3,857 fpm, and the last recorded is at -7,688 fpm. Then nothing.

P/S, Static main issue. Maybe. Hard to believe that Birgenair/Aeroperu comes up again so think of likely non static alternatives.

Load error, possible Static.

You can not load 189 passengers wrongly on a 737. It is the maximum capacity, so that weight distribution is always within limits. The fuel will also never cause any problems either with a full pax load, because of design.
Leaves the luggage. Knowing that indonesians will carry fairly bulky stuff with them,I dont see how the cargo compartments could not be full. This would imply an evenly distributed load among all 4 compartments.
—> this will not turn out to be a load distribution accident. I go for pitot covers as well.

You say that; yet a Malaysian A330 did the very same thing at Brisbane just a few months ago. Airborne without even a V1 call. The pitot covers were left on.

A stall seems unlikely as they rather seemed to be going quite fast. The data from FR24 are groundspeed data and that is of course influenced by wind too.
But still to stall at that altitude with a groundspeed of 320 kts you would probably need winds of over 100 kts so that is very unlikely.
Especially with surface winds of less than 5kts on departure.

I roughly checked the timestamps and distance between a set of coordinates and got an average ground speed of 315 kts. I used about the 5 last minutes of the track, the timestamps are not very accurate so you can't just check between two positions. Anyway this is just to check if one can suspect the ground speed data to be faulty which there is no hint that they are.


Quite the opposite: The ground speed data suggest they were going near top speed.

It's quite easy to compare it to a flight that was flying at a similar altitude and direction / position.

Marked acceleration at 10000ft on this flight might suggest they were flying at 250 kts IAS below 10000 but i don't know local departure procedures so that is speculation.

Anyway the accident airplane was going a lot faster than a similar type at that altitude in the same conditions.
Also in regards to the one engine speculation quoted I doubt they would be able to go that fast with one engine.

With the snag report of the previous flight in mind, it appears to be a case of badly repaired unreliable airspeed/altitude indications and the mismanagement of the re-occurrence on this fatal flight.

What you are forgetting is that they also load CARGO, for eg. Gold or other heavy items!!!!! ....so loading could be a problem, but I doubt it on this flight.
Pitot probes should give wrong take off speeds during take off run as well?

Thanks for that. I guess that I was thinking about it in a very binary way, ie: that either the pitot covers have been removed, or there will be zero or no airspeed indication. Apparently it is possible to have some airspeed indication, even with the covers installed.

The CoG is way forward on the MAX due to the much heavier donks mounted further forward, on a NG a normal load config would 66%aft 34% forward, the MAX needs it all in the back. ( on this point FR will have fun and games with their hold two load policy)

I would be surprised if pitot covers fitted for over night.

Conversion course will depend on individual operators and their governing authority, in Norwegian we have a 3 hour CBT and you're good to go, apart from the start sequence which is akin to the count down for a Saturn 5 launch its basically straight forward once you figure out where the switches have gone. The FMC is v different and could cause issues for the VNAV inept “what’s it doing now” brigade, but how do you lose control of a modern jet in daylight VMC that will not be anywhere near its 82’000kg max take off mass....

Having had to un-train and re train ex Lion air pilots from P2F and listen to the very many stories of incompetence from the LHS I would be very surprised if this is anything beyond pilot error in one form or another, be it missing if fitted pitot covers, CoG or unreliable airspeed.

The MAX is now a mature product, we operated some of the first MAX in Europe out of the box and Transatlantic within a couple of weeks from delivery, I’m not surprised the Australian government have issued a no fly for staff & contractors, did they not ground Lion air some years back when ex bmi flight management became the fall guys.

That was Tiger (now a Virgin Australia subsidiary)

Lion never started an Aussie Domestic operation however they did lodge. They pulled out because of high expected costs.

Every time there is an incident/accident, this place is bombarded with posts made by 'instant experts' based upon ADS-B data and tenuous links to previous events.

There is nothing professional about this. Nothing. It's also wholly disrespectful to those who have been tragically effected by these sad events.

I just wish we could leave this to the experts from the relevant countries safety authorities, accident investigation departments and other relevant stake-holders who are highly trained in such investigations and who are also motivated to get the answers as to 'why' as soon as practicable.

I know this is an open 'forum' but perhaps the moderators might consider some filtering to at least retain professionalism, facts and accuracy from posts made on PPRUNE, rather than drivel and conjecture, when we are faced with tragic events such as today.

RIP to all those lost.

I don´t get it.
You have 3 independent sources for speed and hight. If you have different indications then compare the 3 values. When the CAPT indication is different to IFSD (integrated standby flight display) and FO indication and these two are equal then switch indication and NAV on 2 (FO) and have a good flight.

It's a bit early to state with confidence that the accident resulted from A: Unreliable airspeed indications, and B: specifically, the crew's failure correctly execute said procedures.

Nope.

So if those two indications match at 20 knots you’re going to switch and use them?

You level off using pitch power datums, assess which readout is correct (maybe two are erroneous due impact damage with birds) then make a logical decision using all the information to hand. Methodically.

There is nothing unprofessional or disrespectful about taking the facts that are known, and applying knowledge of aviation to consider how an accident may have occurred. This is not a news organization presenting speculation as fact. If is a forum. Forums are for discussion. We are discussing things.

I agree, I wonder if this is the sort of speed you would reach if you were to put the throttles to the position they should go to as part of the checklist for an air speed indicator failure. We have had a lot of stalled aircraft in recent years, albeit at altitude, but my understanding is that there is a set throttle position & pitch that gives a reasonable guarantee that the plane will maintain altitude whilst diagnosis of the situation is performed assuming good working engines & flight surfaces.

Guys, all ideas about the cause of the crash are pure speculation at the moment. Could be a fault in the engines, pivot tube covers, pilot faults or a big bird ****ted into the engines.

We'll read it soon, latest in the accident report and personally I hope, that they'll learn about this accident. Lion Air and Indonesia in general showing a pretty poor safety record....

Certainly unreliable airspeed indication is one possible cause whcih shouldn't be discounted. We are, however, a long way from being able to say that we *know* that this was the cause. Yes, the fact that the airplane was previously written up for unreliable airspeed is an important clue. But it is in no way proof positive that the problem repeated itself. If it were, accident investigation would be vastly simplified. We could just pull out the maintenance log, find the last problem reported, and list that as the cause of the accident.

If airspeed is unreliable, it does not matter that it indicates the aircraft is going fast.
Unreliable might mean too hig, unreliable might mean too low, for sure unreliable means neither the pilot nor any speculations should rely on it...

Anyway, the aircraft took of around 90 Minutes after sunrise in good weather. Quite different from Birgenair or AF, there was clear visual reference all the time. Good pilots could handle aircraft under such conditions even after full loss of instruments...


It could be flight controls problem.
Is the 737 max full mechanical controls? Or are there some FBW elements in the control systen which require valid air data to work properly?

The "...seemed to be going quite fast" observation is a reference to the "Flight Radar24" data, whcih is ADS-B data, derived from GPS measurements and is completely independent of, and unaffected by any problem with airspeed indications.

If your GPS reported groundspeed is is approximately 300 knots, a stall is pretty unlikely.

Two minutes in to the flight, 334 KTS at 1600 ft, I 'd be interested to know about flap retraction and whether something broke off leading to flight controls issue ongoing and unrecoverable.

did the accident crew know about the technical issues of the previous flight? I know they *should* know since it was written up, but we saw in the Air Canada SFO pre-flight briefing the crew totally missed vital information about the runway closure. Perhaps the same thing here, the accident crew omitted reading the tech writeup, or didn't take onboard the significance of it?

This one is really staring you in the face. Airspeed problem previous flight. System flushed and tested OK on ground.
New flight, loss of control.
Not 100% sure, but I believe a test flight by one of our technical pilots would have been done before the aircraft was released to the line if this snag happened in my airline.

So at least two people above have no grasp of critical angle and what a stall actually is. :rolleyes:
