Aeroflot TU-154 uncontained engine failure.
 
Found this piccy of an Aeroflot TU-154 having suffered an uncontained engine failure that supposedly happened on take off roll. Interesting to see damage to NO.3 housing as well, could easily have taken that out too.

Looking in the inlet it looks like a slipped disk.

Likely it wasn't the extinguisher that did the trick but shutting off the fuel at the pylon

With all the aeration of the nacelle after the failure and ignition, the full fire handle saves the day.

I see the fan hub laying in the inlet with all the blades seem to be missing.

Reminds me of the Anset B727 at BRS only it's the #1 engine on the 154

It surely looks that way to me - with less than 30 kts speed when the incident happened it should not be impossible to have debris thrown that far forward.
