Evacuation Slide Falls From B767 Over BOS Suburb

An evacuation slide somehow departed a DL B76W CDG-BOS over the suburb of Milton, Massachusetts on Sunday while on final approach to 4R. Not sure how this happened, but I have not heard of this previously. Flight # DL405, aircraft N1607B. It appears to have been an over-wing slide.
