Sky Airlines A319 thruster reverser deployed inflight
Incident Sky Airline flight in Lima
In the morning, an Airbus A319 to Sky Airline, operating SKU803 flight to Santiago, suffered an engine failure after 20 minutes after takeoff from Lima Airport.
The flaw, would have generated noise and vibration, the crew safety following the procedure of the aircraft, returned to Lima activating the airport emergency procedure.
Passengers were transferred to another aircraft hours later to continue their journey.

It's in the comment, bottom of page in the link:
"CC-AHC es la matricula y el problema se debió a que se activo en vuelo el reverso del motor N°2 una falla muy extraña en este tipo de avión, de hecho suscito la atención de AIRBUS por lo cual tecnicos de ellos iran a Lima a periciar que pudo haber pasado, ya que el avion quedo AOG en Lima."
meaning:
"Registration is CC-AHC and the problem was caused by in-flight deployment of engine n°2 reverser, a very infrequent failure on this airplane type. This drew attention of Airbus who thus sent technicians in Lima to find out what might have happened, as the aircraft is grounded in Lima"

Only other incidence that I can remember was the Lauda 767 with loss of control and subsequent crash. It will be very interesting to learn more about this - reverser deploying at CLB thrust....
