There may be a clamour for stringent medical assessment, but considering the limitations of the current assessment techniques, including imaging, regulatory medical examiners are doing their best. Yet, nothing is foolproof, and sometimes one could be faced with 'pilot incapacitation' as it happened in this tragic case.
