I've always wondered why helicopters like the AW-139 and Blackhawk don't have higher maximum take off weights (especially for external sling loads). How is the MTOW determined? Thanks

It's the highest weight at which compliance with each applicable requirement of the FAR is shown.

A total weight with jettisonable external load can be established at a higher weight with a couple of provisos including limited operating limitations.
