B212 Category A operations

In the supplement of the operating handbook it states that Category A operations is limited to two pilots. I have seen several companies around the world running who fly category A departure/arrivals single pilot on other multi-engined aircraft types, especially EMS. Is there any reason for not doing the same in the Bell 212 other than the limitation in the POH?
