HELP! Rotary Accident Case Studies Needed.
 
I wonder if anyone out there would be kind enough to let me know if they are aware of any decent case studies that exist of rotary accidents. This is for CRM/Human Factors training, which I am always trying to improve, but getting hold of tried and tested case studies is not always easy. I am trying to get hold of any that were caused by/compounded by loss of Situational Awareness, critical Communication errors, Fatigue or stress. In particular any from the military/uniformed sector. Any case studies would be sanitized. Please e-mail me and I will satisfy you of credentials if required.

You may take a look at www.bellhelicopter.textron.com and search for Heliprops, the human AD.
You may contact them and get free newsletters, they have some case studies and other nice information. also there is www.flightsafety.org (flight Safety Foundation) go under publications and look for helicopter safety, these newsletters go back to about 1990, and have some good casestudies

You may also want to look at the NASA report: 'U.S. Civil Rotorcraft Accidents, 1963 through 1997', published Dec 2000, NASA/TM-2000-209597. I found it on the web but can't locate the URL right now.

It may be more general than you want but may lead you to further case studies.

contact me for some info on two helicopter crashes that I helped with (not the investigations, but afterwards). I don't have the accident reports, but I can tell you what caused the crashes that the accident investigations didn't find or consider.
