Fatal Helicopter Crash in Nigeria
 
A Dauphin helicopter had a fatal accident at Brass in Nigeria on the afternoon of 3 December. Both pilots and 2 of the passengers were killed.
The cause of the accident is still under investigation so please, no speculation.
I mourn the loss of 2 good friends and extend my most sincere condolences to the poor families of all those who died.

AFR: Pilots, two others killed in Nigerian plane crash: official Nigeria crash (LAGOS)
Four people, including two pilots, have been killed and eight others injured in southern Nigeria when a private charter helicopter crashed.
A statement issued by Nigerian Agip Oil Company -- which chartered the helicopter -- states the helicopter plunged into the water near Brass terminal.
On board were 10 passengers and two pilots.
The two pilots and two passengers died, while eight other people were rescued and taken to the terminal for treatment.
The identities and nationalities of the dead and injured are not known.

Its always sad to lose friends in such a manner..........we all know someone who is not with us now, due to the dangers of our industry.

Whilst your wish to prevent spectulation is admirable, it also stifles the learning and education process of Prune........

Considering the fact that it is Nigeria........the facts may never be known.!!!

Can you enlighten us a little........Company??...Offshore...???IFR/VFR....Day/Night...Ground Fire??????....and other revelant salient points.

Despite the fact that it was in Nigeria I think the facts will be known, as the operating company was the reputable Aero Contractors Nigeria and this is their first fatal helicopter accident for over 20 years. The accident happened in daylight, VMC and the aircraft ended up in the water alongside the helipad at Brass Oil Terminal on the coast of the Niger Delta. As, so sadly, both of the crew have died, it may take longer to find out the actual cause, but the aircraft was fitted with a CVR. At the moment, anything further is speculation as this early on in the proceedings there is still quite a lot of confusion.

There are only ideas, some based on speculation. The CVR will, hopefully, give some more facts. I will not speculate, but I am sure that there will be some news soon. Communications from Nigeria always take time, but I am confident that ACN will release information once they have completed their investigation.

It is well over a week now so maybe it would not be insensitive to give the names of those involved.

Aircraft now at Agip, gather the engines have already been sent out to Europe, aircraft to follow soon.

The funeral of Mike MacFarland will take place in Aberdeen on Monday 20 January. Eugene Attere has been returned to his family in Benin Republic.

I don't think that any of the helicopters in Nigeria have HUMS fitted, (with the possible exception of the Shell EC155s?) and many have neither CVR nor FDR. I think that the only requirement is that if a CVR is fitted it must be serviceable!

There are definately some (5?) 212s with Integrated HUMS (ie plus FDR & CVR).

The EC155s were ordered with a new type of EC HUMS (which was retrofitted on to the first 2 I believe and from new on the rest).

Surely we must have a substantial idea as to the cause of this accident.........

Despite the clamour for information, the problem is that there is no official news as yet. I heard that an engine failure was reported at the time of the accident. Turbomeca are involved in the post-accident engine investigation. It has not been confirmed if there was any other emergency at the same time. With the unfortunate loss of the crew, the 2 main witnesses are lost. Bad, or uninformed information, is probably more damaging than no information and I'm sure that when something more is known it will be reported in public. It says something that the fleet of Dauphins is continuing to fly as normal.
Just be patient, when something relevant is known, I'm sure someone will report it on PPruNe

Haven't the Dauphin 365N2 fleet at Aerocontractors been grounded pending findings from the investigation teams??

One hopes that nobody is still flying these helicopters around until the results of the technical investigation surface.

With reference to this accident ECF has issued a reminder that the tail rotor pedals on a N2 are not to be operated with the rotors stopped and that the tail rotor blades are to be inspected iaw a current service bulletin.
