I don't know from where you get your info, but there are at least two FAA level D FFS in the USA, a S-76B/C+ at CAE in NJ, and a level D S-76C++/ D in West Palm Beach at FSI.
I am not sure whether there is an additional one in LFT.
There are other S-76 sims in China and Europe.

Sorry for the wrong data.
The FAA's National Simulator Program (NSP) does not list them in the devices spread sheet where I looked, but does in the program managers by company spread sheet.
That sheet does not list the level of approval.

FSI's 76B sim was pretty good, when we practiced losing an engine on takeoff from East 42nd St helipad, pop the floats and sploosh into the river, it rocked gently with the waves and made gurgling sounds. (Might have been the instructor throwing up, though...)
