This looks pretty real, I have seen more and more of peoples bodies suspended outside of the helis for these sorts of non-chalant selfie shots.
I used to catch people sticking their feet out of the heli and taking photos of them for their Instagram feed, especially on the photo charters that I flew. This of course was after specifically giving them a very detailed safety brief before the doors-off flight never to do such a thing. They usually stopped after I yelled at them.
Playing devils advocate here though; it would probably be said that he had the lap portion of his belt still secured and that it was pre-planned to remove the headset and carried out at a very low airspeed etc.

Granted that this is not much different from when you'd have troops sitting in the door frame with their feet on the skids.

I remember doing this with the Super Pumas in the SAAF. 3 per door.
Messes with your head the first time they turn but then you remember the whole bucket of water swung on a string thing and relax the death grip on the guys next to you.
It's worse on the outside of the turn with blue skies, you get sudden vertigo because it feels like you're still banking without a visual reference, as you can't see the horizon.

Guess that the biggest zero has just been given a helicopter ride
