I guess it's something to do with the pressure of high office and dealing with weighty issues (possibly more than one).

Why is the chap on the right asleep?

Just noticed that despite the additional weight of the medal on the left breasticle, the right breasticle is subject to more static droop.

no the left one has more droop !! from his point of view and mine
