When you read all the report, it makes very sad reading, whilst I do not know any of the people involved I sincerely hope none of us Ppruners ever have such a sad occurence, that affects so many members of one family!
The reasons for the incident seem straight forward enough, we are all warned often enough about this possibility, Do we all listen?

The wreckage of that particular machine was still in the AAIB hangars when the CAA Helicopter Safety Seminar visited some weeks ago.

I must admit it took me a while to work out it was actually a helicopter in the first place, and I only realised what it was when I read the identification sheet in front of it.

Even to someone who doesn't have technical competence in this particular area, the report's conclusions are all too transparent, and should be mandatory reading for anyone learning to fly - fixed wing or rotary.
