Fatal Heli incident
 
I've been asked about an incident involving a helicopter, where the pilot/pax lifted a child into the rotor disc after landing with fatal results.

Possibly occured in the UK, in the late 1990's...might be an urban myth.

The AAIB website isn't the easiest to search, Any ideas?

Not an urban myth.
It was a R44. Robinson use the accident in their Safety Course Seminar as why the pilot should always remain at the controls until the blades stop.

Similar type incident here, which involved the pilot being hit by a rotor blade, this wasn't fatal although it did involve some very serious injuries.

Years ago here in Canada there was a rotor disc accident involving a passenger. The ship landed and the passenger left the chopper while it was running in flight idle - I think it was a 206. She walked away from the machine and up a hill right next to the helicopter. The next thing the pilot saw was her head rolling down the hill.
