Insecure storage of a simple canvas bag caused the crash of a Hydro One helicopter near Tweed almost two years ago, killing all four people aboard, the Transportation Safety Board of Canada has ruled.

Wow......reminds me of a very similar incident to a Jetranger in the UK back in 1996. The rear baggage locker opened in flight and a Jacket flew out into the tail rotor causing complete failure. In that case luck was with the pilot as they were over open countryside when it happened. He managed to put down in a field and all survived. Report Here
