Ladies and Gents, how does Air Navigation chart and database providers (i.e. Jeppesen) create charts and databases worldwide?

See ICAO Annex 4.

I'm not being brief because I want to be unhelpful but simply because I don't know much more - but that document will tell you everything and more!

Every country has to publish an aeronautical information publication (AIP) on a prescribed basis. All the instrument procedures are in the AIP.
