EASA CAA to FAA conversion.. Sorry in advance :(

I know this has been asked a thousand times before and trust me i have read through most of the articles regarding the subject, however ...

I have to download and complete form 'verify61.pdf' from the FAA website however all URL links pointing to the page that contains this now directs to 'Page not found'

I have tried searching on the FAA website but i cannot find what i need.

Please could someone assist me as Im getting pretty close to the 90 days minimum period required for the licence to be converted.

Also, just to confirm, the procedure goes as follows...

Form from CAA + fee
Form from FAA + fax it to FSDO nearest to where i am staying in the USA (chandler Az)
Tell FSDO dates to which ill be staying to try and speed up the process.
Visit FSDO for some checks and pick up my 12 month valid FAA licence

Take FAA PPL to an international CFI , do proficiency check and get it stamped.

Job done??

Many thanks for your help

Ryan

You need to have your foreign licence verified by the FAA.

The below link has the instructions of what must be done. It also details additional information required for UK CAA issued licences.

Airmen Certification ? Verify the Authenticity of a Foreign License, Rating, or Medical Certification

UK CAA:- Complete the form allowing them to release your information to the FAA (SRG1160) and complete the payment form (SRG1187). Send this back to the CAA by post.

FAA:- Complete the Form "VERIFICATION OF AUTHENTICITY OF FOREIGN LICENSE, RATING AND MEDICAL CERTIFICATION". Sign and scan and email back to the FAA. The email address is in the link above.

FSDO:- Contact the FSDO once you get a reply from the FAA to say all is well and schedule the appointment. They should tell you the exact information they need, licence application, log book, medical etc.

FSDO:- Turn up at your allotted time and get your temporary airman's certificate. It is valid for 60 or 90days or something like that.

The plastic card will be posted to your home address.

Take your BFR.

Go flying....
