PPL Verification question's

Hi Guys,
Im going to hour build in Florida in the next few months and i just need to make sure of some things. http://www.caa.co.uk/docs/33/SRG1160FFenabled.pdf
Is this the form required for the CAA and this http://www.faa.gov/documentLibrary/m.../ac8060-71.pdf
for the FAA? Im a bit confused on how to do this... Can anyone help me? I will appreciate it... Also my SEP on my PPL has expired (haven't done the 12 hours in 2 years) will this be a problem or should i renew my SEP first and then complete the paperwork?

I am sure if you do a search that you will find numerous threads confirming the procedure.
Also my SEP on my PPL has expired (haven't done the 12 hours in 2 years) will this be a problem
It will if you want to fly; without a valid rating you cannot exercise the privileges of your licence, or one based upon it!
