No surprises there really. I knew about the lack of NVFR recency but was hounded for saying it on here in the last thread.
In the synopsis alone there is far too much use of the word "probably." |
via VH-XXX: In the synopsis alone there is far too much use of the word "probably." The ATSB found that the pilot probably selected an incorrect destination on one or both of the helicopter's global positioning system (GPS) units prior to departure. The ATSB concluded that, after initiating the right turn at 1,500 ft, the pilot probably became spatially disoriented. Factors contributing to the disorientation included dark night conditions, high pilot workload associated with establishing the helicopter in cruise flight and probably attempting to correct the fly-to point in a GPS unit, the pilot’s limited recent night flying and instrument flying experience, and the helicopter not being equipped with an autopilot. The thread should probably remain here as the NVFR discussion has implications for fixed wing too and not everyone reads rotor heads.
via VH-XXX: In the synopsis alone there is far too much use of the word "probably." Hmmm... i'll bite. Why do you think that VH-XXX ? In the US NTSB reports, if they can't find a reason or don't definitively know, they state this and then just say what probably happened. Look at the graphic on page 4. They lasted 171 seconds. Sounds familiar? Who remembers "178 seconds to live?" Link not working - try this one. |
