Probably apocryphal, but after a British captain landed at the wrong airport decades ago (may have been on one of the several parallel runways on Singapore Island), ATC asked him what his intentions were. Answer:

'A chicken farm in Surrey'.

It has got shades of the various Essendon v Tulla mishaps over the years and just like gear up landings will continue to happen while us mere mortals continue to peddle airborne tin and plastic around the place..

And FP's possible scenario on this rather embarrassing incident :
It’s not entirely clear what happened, but here’s a plausible scenario: After passing the IAF at WITBA, the pilot began a descent, nominally to 3000 ft. The pilot knew he was on an RNAV (GPS) 19L approach, because he read back the clearance that said so, but may have been thinking about a visual approach. Indeed, the recording shows that he started to say “visual approach” rather than “RNAV (GPS) approach”.

At some point, he saw the runway environment of KAAO, which has the same alignment, more or less, as KIAB. Seeing what he expected to see, he discontinued the instrument approach, and flew the approach visually. It was only much later, after he landed, that he realized that he was still short of WARUN, much less KIAB.

The pilot was cleared to fly an RNAV (GPS) approach. That approach clearance requires that he maintain at least 3000 feet until passing the FAF, WARUN. Had the pilot flown the instrument approach as required, there would be no possibility of mistaking KAAO for KIAB, since the approach required him to fly at 3000 ft MSL over KAAO, 1600 above ground level. Upon reaching WARUN, neither KAAO or KBEC could be mistaken for KIAB. So it appears that the pilot simply neglected to fly the approach he was cleared to fly, and instead flew what was essentially a visual approach. Unhappily for him, there are two airports with the same runway alignment along the final approach course. To make matters worse, he failed to have the situational awareness to understand that he was 7 miles short of the correct airport.

Yes Dart, a chicken farm in Surrey could be a better career choice after this.

Surely it was a two pilot op..!? The CVR would probably show that they decided to discontinue the approach and continue visually with rwy lights in sight somewhere prior to the fix..after that its a visual illusion/confusion issue at night..also maybe didn't properly brief the other airfields in the area..?

After this sort of mistake are pilots in the US invited up to the FAA offices for tea and bikkies with a reminder to bring a large jar of vaseline ?

What I'd like to know, if it happened in Australia, would it be a strict liability offence, prosecuted, lose your flying privileges, pay a large fine & find yourself looking at time in the big house?

...............that depends 'JR' on whom you know, remember CASA are made up of friends of friends, mates of mates in the industry, like all things in life humans are notorious for looking the other way when it's necessary.

The eyes are very powerful information devices & will override all other sensors going to the brains CPU. The drivers 'saw' what they wanted to see, all other info was then disregarded & the focus was to land......land Mavrick you are instructed to land..........-)

IIRC a couple of crews have tried to land on the Bruce Highway/Nebo Rd in Mackay over the years..!?

In the USA, punishment will be up the Chief Pilot, with possible unofficial input from the FAA. More often than not, it will be a few weeks on the beach, and then a return to duty with nothing further.

If the CP is a real horse's rear, it will be termination, but I've only heard of that once, in the case of NorthWest Airlines landing in Brussels instead of Frankfurt.

It's a classic, even an easy mistake, done many times before, like the gear-up landing.

I've had the pleasure of saying, a few times in my life, "Weren't you the one that said you'd never ever make that mistake?"
