Swiss cheese?
 
I realise that any connection with military aviation is non-existent. But this recent report on an incident on the railway struck me as a beautiful example of the Swiss cheese model at work. How many chances to avoid the incident were missed?

Um...holding on a large frontline RAF Base, I was tasked with updating the Station Inventory. Two thirds of the HAS sites and one fuel dump were not on the Station plans or Inventory. I was issued with a station bicycle and a tape measure and got to work!
On the plus side, at least they had an inkling things were wrong, hence my being given the job. Perhaps Hackney Council or Network Rail needs a few holding officers.

Hundreds of holes were being drilled at intervals along a highway to determine the depth of construction, underlying soil types, etc prior to reconstruction.

Found a really hard spot then the drill suddenly broke through into open air!

Operators looked around and realised they were standing on a bridge ....

You need more than just drawings and paperwork to stop this sort of thing happening. :ugh:

Ahh that's chicken feed, if you haven't seen the documentary of this on the History Channel watch this,



They were drilling in a lake and hit a salt mine, basically the lake drained into the mine and the canal that flowed out of the lake to the Gulf of Mexico reversed and formed a 150 Ft waterfall as the gulf flowed back into fill the lake ( that had been 10 ft deep ) and the mine..
