It was ruled a "preventable" accident. Assuming that's a fair assessment of the cause, then sacking sounds appropriate...

Came across this series of ' Life aboard a Los Angeles Class Fast Attack Submarine ' on YT
Found it interesting and is a series of 7 videos about as it says.... one of the ep considers Sonar so enjoy

The Navy has been happy to relieve COs, during the last decade or so, for less.
