My former employer offered BUPA which included U.S. coverage. The U.S. part of it was covered by United Healthcare.
For travel insurance for U.S. I use world nomads with very good service.

Health insurance
If you are flight deck crew Try Baymac . There seem to offer a cost effective solution for you and the kids are covered for free. They also have a plan for students 19 -23. If you are in UAE they have a local contact who can contact you in person. I personally don't have any experience with BAYMAC
