Boredom on the flightdeck
 
Does anybody know of incidents/ accidents that have occured due to boredom on the flightdeck (other than those cited in the Naked Pilot). I.e. pilots playing with switches, altering configuration unnecessarily etc. Or maybe lack of concentration due to reading the newspapers instead of approach plates.

This info is to form part of a presentation I am due to give as part of my ATPL
groundschool training.

You wonīt come up with much concerning "boredom" - especially looking at the scenarios mentioned.
Try to make a change to the subject of "complacency" especially in hi-tec cockpits and you should get quite a bit of information on the net.
Sorry I canīt offer any links - would have to do the searching myself.

If memory serves me there was a DC-10 in the US in the 70s where the crew were playing pull the CB and find out what it really does. Anyhow trashed an engine I think, (all a bit hazy really)
