Preparing for class 1 medical - gatwick

Hello. I've got my class 1 medical coming up very soon and I was wondering if there were any forms to fill in before going there. I recieved the letter but they failed to attach the instructions on the day with it. So how do i prepare for it and also could some one tell me the full address of the place i need to go. Thank you for your help.

Download forms 160, 162 and 163 - follow the guides that are on that page to fill them out.
