When can and when can't passengers fly?

Does anyone have a definative guide?

Clinical judgement is helpful, but I was wondering if there is anything more "robust" available, to guide clinicians when advising patients.

I'm looking for guidance about passengers flying in a pressurised cabin.

IATA Medical Manual (not downloadable from the Internet as far as I know), Page 55-58 lists the specific medical guidelines for certain conditions.
