3rd week?
 
So, is it now 3 weeks worth of newsletters in which we have neglected to mention the 2 engines that were "fried" on start-up in MIA?

Surely $US40 million has nothing to do with the complexity of the fleet we fly..... Maybe we should also start flying the heavy twin just to add a challenge?

If you look up the crew of that day you will realise why it hasn't reached the news! Poor chap thought it was autostart, put the fuel on both and didn't even monitor!

No, we only learn from the mistakes of the peasant classes. If the ruling classes screw up it's a system problem - remember RF and the gear pins left in after HIS walk around.

I agree, with so many variations and permutations on the fleet, this was absolutely going to happen! It was just a matter of who and when! In fact I've seen an STC nearly do the same with a Rolls. Poor buggars, there but for the grace of god and all that.....

The Swiss cheese lined up, and not surprising. To many variants and minimum training, throw in checkers that expect you to redesign the aircraft by memory, and it's bound to happen. Amazing thing is the biggest mishaps is by management and trainers with sharp pencils. Just the CX culture catching up.

The secret is Senior Training Captains are better than the average line Captain. They never make a mistake but hell if you ever miss a call or a minor infraction on a line check.

Mind you that forward cargo fire warning is just an indication problem so it is ok to pass suitable airports (Boeing 777 Team).

We have been very lucky at CX not to have an accident but when does our luck run out?

Don't let it happen on YOUR WATCH. The most benign thing can turn into something very bad in a matter of seconds.

Anyway so what’s the duff on what happened 3 weeks ago? A visual approach, a messed up VNAV/ MANAGED Approach? Flying through a thunderstorm?

Did it really happend? The same rumor was floating around 6 mos ago, including MIA where it supposedly happend.

I heard it happened in ANC 6 months ago but no engines damaged although a lot of smoke during the cycling.;)

If this actually did happen, there is nothing these guys should have been confused about.

Out of 37 B744s (Pax and Freighters) and 5 B748s a total of 11 aircraft have Autostart fitted....

Even when Autostart is fitted, monitoring the start is a must.


In any case I doubt this happened...

Could just be a re-spin of another airlines incident at LFPG in the 90s.

Crew had just taken delivery of a brand new -400 in Paris (a few days after the delivery flight from Everett), and had to date been using another airlines -400 sim fitted with autostart.

IIRC They started 4 and 3 together and were clueless as to why both motors went to ****. (No autostart fitted).

Just like CX...Penny wise, Pound foolish :E

The only reason the 6 ERF's have it is because it became a standard and no longer a customer option on the ER/ERF. Same reason why they have taxi lights, etc. etc.
