From Flying PAN to Frying PAN. Next sim check should sort out the procedure...assuming he gets the checker who is current on gear pins (well circa 2001).

If the cap was PF for the sector he isn't the one checking for no pins on the walk around. So ultimately how much trouble would he be in.
of course the pilots will get blamed and there could be no other reason because our SOPs are perfect

If my memory serves, the previous incident was the "Fleet Manager" and he did the walk around. Even got promoted after that I recall..

Let's not forget the ground engineers in all this. The AML will have stated the pins were in and should have been signed off when they were removed. Often I see this signed off BEFORE the pins have been removed to expadite the ground engineers exit in which case I refuse to sign the commanders release until they have been removed and I have physically inspected same myself. It has caused a few delays but hey ho.

A340 gear pin incident a few years ago involved a pin with no streamer - at the time, engineering were still using streamer-less pins on occasion. Quite possible that same featured in this event.

Streamer or no streamer the AML will have stated pins installed then removed. This should have alerted the crew to make doubly sure the pins ( with or without streamers ) were OUT. No excuses here. Someone f**** up. Own it.

Ex MIA, Central American airliner taxiing behind an A******an Airlines jet.
"A******an on taxiway A, this is TACA 123 do you read". Twice called.
MIA ground. "A******n XYZ TACA is calling you".
A******an XYZ. "We professionals at A******n and do not indulge in idle chit chat on frequency".
TACA 123 "Miami ground please tell the professionals at A******an that the streamers that say Remove Before Flight are still in"
