1. Telephone US Embassy for an appointment for a nonimmigrant student visa (M-1) on 0904 245 0100 -- calls are charged at £1.20 per minute.

2. Complete form DS-156 (Non-immigrant Visa Application) @ https://evisaforms.state.gov/ -- print completed version & retain for interview

Photograph requirements: http://travel.state.gov/visa/temp/info/info_1287.html

3. Complete form DS-157 (Supplemental Non-immigrant Visa Application) @ http://www.state.gov/documents/organization/79964.pdf -- print & retain

4. Complete form DS-158 (Contact Information and Work History for Non-immigrant Visa Applicant) @ http://www.state.gov/documents/organization/79965.pdf -- print & retain

5. Make the I-901 fee payment to the Student and Exchange Visitor Information System (SEVIS) @ http://www.fmjfee.com/ -- print receipt & retain

6. Checklist of documents for interview:
  • Passport or UN Travel Document
  • Non-immigrant visa interview appointment confirmation letter
  • Machine Readable Visa (MRV) fee receipt
  • I-20 M-N Certificate
  • DS-156 (including regulation photograph)
  • DS-157
  • DS-158
  • SEVIS I-901 fee receipt
  • Bank statements to prove financial support during stay
  • Travel itinerary (either intended or booked)
  • Sufficient funds for Embassy courier service
  • & Dependents to be admitted under M–2 status (if any)

Now ask yourself why you're throwing money at a company which leaves you asking this basic question on PPRuNe.
