1985 B747 accident ground loop with No 1 full forward thrust
Air France Flight AF091 - Aviation Accidents Database
It might be an accident from yesteryear but well worth revisiting. 747 touches down normally. Full reverse on all engines selected. But No 1 engine has an undetected reverser system fault and goes to full forward thrust and it took critical seconds before the fault was recognised. Resulted in ground loop and fire as landing gear collapsed.

I didn't start flying the classic 747 until two years after this accident . If my memory serves me correctly the FE used to call any abnormal reverse indications and I wonder if this was introduced due to the accident.
Another accident caused by the same "failure".

Interesting story of Boeing grafting a new nose onto the aircraft on the island. Had to wait for the sun to go down prior to doing any drilling/riveting work so the sun imposed bend in the fuselage would straighten.
