Scathing ACCIDENT Report,indeed.Unique insight with the whole sequence being visually recorded from the jump seat. PNG/PX now has no effective Pilots Union to oppose the"proposed" introduction of Flight Deck Video cameras,so ,likely it will happen.Sure makes the Accident Investigators job much easier! PX MD/CEO/Boss Man, (or whatever his title is,) is still calling this an INCIDENT, & Human Factors ??? are to blame (ie NOT Pilot Error) (in todays Pos Guria).He may need to brush up on the Definition of Accident vs Incident.Sad outcome for a once Great Little Airline.

Anyone know if the aircraft was ever recovered or has it become an artificial reef?

Check out section 1.12 of the report ("Wreckage and impact information")... "recovery" could be a tricky prospect.

"To a large extent the rear fuselage remained connected to the forward fuselage by plumbing, cabling, and some minor structural components."

Did they ever salvage the wreck, or is it a permanent artificial reef now?

No photos have ever surfaced (geddit?) of the aircraft being recovered, so I think we can safely conclude that it's still there, 30 metres down.
