SID RT Phraseology
 
The UK require a/c to check in on their departure frequency with

Callsign, SID designator, passing altitude/level, cleared level.

Is this just a UK requirement or is it European requirement?

Just the UK. Other countries / airports will require different information.

Same goes for aircraft type and ATIS on start-up and arrival - it isn't always necessary.

Maybe it's time ICAO looked some at global standardisation here.
Although I hate the term 'best practice', its application just might prevent someone, somewhere, from 'busting' their level, which can only be good.
