Monitoring Guard

It makes obvious good sense, but can anyone tell me where (if?) it is mandated anywhere that airline crews should monitor 121.5? I have Googled like mad but cannot find any reference.

In the states I monitor guard, SE Asia, no, they use it for a chit chat frequency. Sorry I can't help you with a reference, not mandatory.

Not 'mandated' as far as I know but often in Ops Manuals and good airmanship.

you can find the respective SARPs in 5.2.2.1.1.1, 5.2.2.1.1.2, 5.2.2.1.1.3 and 5.2.2.1.3 of ICO Annex 10 Volume II.

Frankly I only wish that people would monitor the ATC frequency!!! It is tiring having to make 2-4 calls before a response is forthcoming

Having said that... with all the military activity through the Gulf at present I suggest also monitoring guard is essential

Airlines generally issue special instructions for this when operating near war (or non-war) zones.
