Class 2 Medical certificate (Uk CAA) medical records ?

Hi I am doing a UK CAA class 2 medical soon, can someone advise if they access your medical records? Or do they go off of the info you provide?

If you withold or lie you risk a criminal conviction
And if you crash you will be uninsured and your family will suffer for decades.
So I dont know why you are asking because you will disclose....wont you?
