To be honest, based on the ground track shown in the report, it almost seems like they misidentified runway 22 for 16L. The report doesn't seem to make mention of whether they discussed the circling lighting that is available for crews to use. This circling lighting includes a row of five lights to aim for and fly over at which point one starts the 180 degree turn to final. There are also reference lights at the threshold of 16L for the pilots to see while on downwind and a light beacon halfway through the turn to final. Being rushed by not discovering that this circling approach would be used until fairly late could contribute to missing this. It is quite possible that this approach was never previously reviewed to any extent or at all due to the perceived lack of likelihood that it would ever be used.

It can be very helpful when a circling approach to the left is flown, to have the F/O fly with the captain giving commands for headings while looking outside(and monitoring inside as appropriate) until taking over upon turning final if there is a requirement or personal desire for the captain to land a circling approach. In this case, the F/O was the one using his ND for reference which appears to have been done poorly and it is quite possible that he couldn't see the airport at all.
