With reportedly high chop rates on sim assessments what have been peoples general findings of difficulty (eg general handling of a new type and flying accuracy, CRM with strangers and ineffective PM role etc)?

I would say:
Poor Preparation
Poor MCC Training
Lack of Interest for the airline they have applied for
Underestimating the assessment process
Language difficulties

The majority of sim rides involve hand flying using raw data. That requires selection of appropriate thrust and attitude together with accurate TRIMMING. Whilst the MCC/APS is mainly flown with the automatics, there are exercises in part involving hand flying.
If you aspire to a certain airline, choose a MCC course with the same or similar simulator.
The bottom line is can you set an attitude and TRIM?
If you can, then you will succeed. If not, get practising on an appropriate simulator.
