, Business & Commercial Aviation, or B&CA, a renowned US aviation magazine, is known for its excellent flight safety articles. The July 2018 issue includes "Cause & Circumstance" a regular series by Richard N. Aarons.
The title of the article is Fatally Flawed Flight Training - when the instructor is a hazard. It described a Beech B-60 Duke that crashed and burned during stall recovery practice. On board were an experienced private-pilot owner and a 90 year old 20,900 hour flight instructor who seems to have been directing non-standard and unsafe stall recovery practices. That included failing one engine at the instant of stall recovery causing the aircraft to go into a spin as high power was introduced during stall recovery.
The same issue of B&CA includes another article Know Your Thrust Reversers, adding that reversers have direct influence on safety and operating costs. That article discusses an accident to a Boeing 757 that ran off the departure the runway and came to stop in deep snow. A defect in the thrust reverser system caused a lock-out while the thrust reversers were in transit. This lockout would prevent movement of the thrust reversers until about five seconds after a pilot moves the reverse thrust levers back to their stowed position, allowing the T/R system to deactivate and begin deployment again when commanded.

Some flying schools subscribe to B&CA for its flight safety content which is far superior to most aviation magazines in Australia. For $79 a year for 12 issues, IMHO it is the best value for money a pilot can buy.
