When did BASI stop putting the flight crew names in their reports?
My guess would be that names were no longer mentioned once we became sufficiently enlightened to understand the Swiss Cheese metaphor and that "naming and shaming" individuals not only had zero safety benefit, but was often completely unjust.

When did BASI include the names of flight crew in the reports? At one point the ATSB even stopped including the name of the airline but that has changed presumably because the rego gave it away.

Folks,
Not "naming" the airline was rather pointless when BASI (in one case) referred to a "B767-238 operated by an Australian international airline".
-X38 being Boeing's constructor number for Qantas.
