The requirements are the same of converting a FAA licence to EASA (JAA is dead, it's EASA now).
You need to pass 14 ATPL exams and convert your CPL and ME-IR to EASA. Then you will have to transfer your 737NG TR to your brand new EASA ATPL but I don't know the process for it. No six touch and go's required given your experience though. I am fairly sure about this.

I'd factor at least €10.000, but given your experience you might be able to study for your ATPL exams on your own with no need of an ATPL provider. This should lower the cost.

i need to know the requirements, the whole process and the average cost ! Best way to get this figured out, is to get in touch with an EASA FTO (flight training organization) who has experience with the conversions of Non-EASA licenses. Check out the links below. But as already mentioned above, you'll have to take all 14 EASA ATP exams and that's the toughest part of the conversion, quite an extensive affair.

Bartolini Air EASA Flight Training Organisation - Fly in Poland (http://www.flyinpoland.com/)

Bournemouth Commercial Flight Training (http://www.bcft.aero/)

Good luck!
