I recall many years ago seeing a quote from a senior RAF or RFC officer dating back to World War I which said something along the lines of "Pilots are forbidden from making mistakes and should be punished if they do" in response to aviation accidents.

Can anyone please point me at what might be the the original and who said it?

I do not know....but this is one of my favourites:

We had an AOC, ( 1960's RCAF Air Defence Command), who put out a bulletin that said that there were no acceptable excuses for a taxiing accident, and apparently he meant it.
