Sorry if this opens a can o' worms, but I'm curious about what happens to flight crew who're found to be at fault, causing an incident/accident (I'm not going to get into a discussion about the differences or definitions between incident/accident). This is prompted by the EK tailstrike, where it appears the crew input the wrong weight figures into a computer and took off with the wrong thrust set. I'm not criticising them or anybody else here, or making any comment about that airline, just curious what the procedure is, particularly where nobody's hurt but it's obvious something serious happened? Is it all handled by the airline and down to their policy, or do the regulatory agencies take a hand and thus affect their licences etc? If they lose their jobs would another airline take them? (I'd be be happy to fly with the same crew, it's a mistake I bet they will NOT repeat!)
