How do you meet the cross country requirements if you are training in the caribbean?

Do you have to literally fly to another island with international flight plan, passport, floatation gear and landing fees in tow?

Probably so. They have to meet the requirements. Some years ago there was no ILS in Jamaica. This meant that all Jamaican pilots had to do their initial instrument rating outside of their country. Miami was a popular place for this. Presumably the same applies to other island states that do not have an ILS.

Most of those pilots train in the USA anyway , just a hop over the water

Yes, that's the only way to do it. In ANU they fly to SKB or PTP, with occasional round robins ANU-SXM-SKB-ANU.
