Interesting Hilico, thanks for the links. As a training PPL I can't really understand how the pilot seemed as if the right yaw of the R44 was unexpected? I'm guessing that someone of his hours doesn't have any experience on european machines and so would not once have pressed right pedal on take off?

No problem - many aeons ago, the mods thought it would be a good idea if the forum had links to the regular monthly bulletins for various countries. I volunteered to do the AAIB. All it is, is checking their site in the second week of the month waiting for that month's edition to appear.

About the R44 incident, I couldn't say (I'm not actually even a PPL), but I have seen someone experienced in American machines push the wrong pedal on TO in an EC120. He was apologising before I'd even have noticed the slip.

The S-76 smoke in the cockpit incident was very nasty indeed and well handled by the crew. Having discussed it with the crew, they were close to losing all visual references due to the smoke.
