1st Class medical in Costa Rica

Hello! Does anybody know who I might be able to talk to about getting a 1st class medical in Costa Rica? I'll be going down there in a month or so and would like to set up an appointment.

Three are listed for FAA First Class, two in San Jose, one in Hereida.

Actually I am looking for a Costa Rican first class.....Is that DGCA?
