Hope I'm not the first one to have had this problem.

Go to the <a href="http://www.hawaiiaviation.com" target="_blank">www.hawaiiaviation.com</a> website and download the application form. Follow all the listed instructions on the form. Then try and fill the form in and save the data. Can't for the life of me get the data I enter on the form to save.
If someone out there in pruneland has had any succes at this please let me know. It's frustrating the hell out of me.

The forms are .pdf (Acrobat) files. Unless you have the Acrobat Writer you won't be able to save them. The free Acrobat Reader has all the save/send options disabled. Since I do have the Writer I can't check this out, but you ought to be able to print the completed form from the browser page.
