Hurrah
It's only taken a timescale measured in years since the KLM/Air France merger. But today I received my shiny new Flying Blue card with my correct name printed on it.
I was looking to update the post I made here in July last year, but the thread has been locked.

Anyway, that's all I wanted to say. And it shows that if you keep at them long enough they will eventually realise that the customer is right when it comes to getting their name correct. I'll be able to finally use the self service kiosks and lounges once again without the hassles.
