Medical renewal

How long after a Class 1 medical expires do I have to have an initial exam again.

e.g. If my medical expired today, and I renewed it in two years time, could I do this, or would I have to have an initial again?

I don't know the exact answer, but I was surprised to find that when I went for a Class 1 recently I only had to have an "extended renewal" (costing about £200 instead of about £400). My original Class 1 had been issued 9 and 1/2 years previous!
